Child Watch Volunteer Service Form

I,

hereby agree to serve as a volunteer in the Child Watch Program for Special Ops Child Program for Special Ops Child Watch Service. I understand and acknowledge the following terms and conditions:

1. Volunteer Services:

   1. I will provide assistance and supervision to children participating in the Child Watch Program.

   2. I will follow the policies, procedures, and guidelines set forth by the childcare program and comply with all applicable laws and regulations.

   3. I understand that my role as a volunteer is to support the childcare staff and ensure the safety and well-being of the children in my care.

2. Volunteer Commitment:

   1. a. I will arrive on time for my scheduled volunteer shifts and fulfill the agreed-upon time commitment.

   2. I will notify the childcare program in advance if I am unable to attend a scheduled shift or if I need to make any changes to my availability.

      
      

3. Confidentiality and Privacy:

   1. I understand that as a volunteer, I may come across confidential information about children, families, and staff members.

   2. I will maintain strict confidentiality and will not disclose any personal or sensitive information to anyone outside the childcare program without proper authorization.

4. Child Safety and Well-being:

   1. I will prioritize the safety and well-being of children in my care at all times.

   2. I will follow the established safety protocols, including proper supervison, maintaining a clean and child-friendly environment and adhering to health and hygiene practices.

5. Professional Conduct:

   1. I will conduct myself in a professional and respectful manner while interacting with children, parents, staff, and other volunteers.

   2. I will refrain from engaging in any form of physical, verbal, emotional abuse, neglect, or harassment towards children or other individuals in the childcare program.

6. Reporting Concerns:

   1. I will immediately report any concerns, incidents, or accidents to the designated staff member in charge.

   2. I understand that I have a legal and ethical responsibility to report any suspicion or disclosure of child abuse or neglect to the appropriate authorities as mandated by law.

7. Termination:

   1. The childcare program reserves the right to terminate my volunteer services at any time if I fail to comply with the terms and conditions outlines in this agreement or for any other justifiable reason.

I have read and understand the terms and conditions of the Child Watch Volunteer Service Agreement. I agree to abide by these terms and fulfill my responsibilities as a volunteer to the best of my abilities.
